PRE-QUALIFICATION OF CONTRACTORS NOTICE.
Separate applications are invited forthe pre-qualif cation of contractors/ firms who had been enlisted/renewed with 11.113 & PHE Department for the year 2013-14 having excellent reputation in the construction of Water Supply& Sanitation Project to undertake the project 50.00 Milton and above:-1. "Construction of Storm Water Channel & Provision of Tuff Tile and Hujra Shah Muqeem District Okara" having approved cost Rs97.030 Million. 2. "Pre-qualification contracting firms will be governed carried out under P&Dguide line. The willing contractors/ Firms falling in the category of estimated cost of the project are required to submit the application alongwith the following information/ documents for pre-qualification for the year 2013-2014 upto 04-02-2014 at 3.00 PM in the office of the Executive Engineer, Public Health Engg. Division, Okara 1. Name of firms/ contractors, partnership deed,/ articles of association in case of company/ limited firm, complete addressand telephone No. ot persons with power of attorney in favou r of persons authorized by the firrns/ contractors. 2. Attested copies of enlistment/ renewal for the year 2013-14 with HUD & PHED in category 'C-2' and Registration of Pakistan Engineering Council in specialization Category CE 09 (Water Supply & Sewerage/ Drainage Project)for the Calendar year 2014. 3. i. Year of establishmentof fimi/companywith proof. ii. No. of Similar nature of works, cost of each work (equal or greate0, scope of work with verification from conce med Executive Engineer/ Project Engineer. iii. Name of similar projects already com pleted during last three years with the completion certificates from the concerned office and number of projects presently in hand along with their magnitude and period and report about quality& paceof work from the concerned Department. 4. Proof of financial standing duly supported with documentary proof/ bank certificate. (Balance sheet duly certified by the concerned bank officer be provided). 5. Proof of Reg isration witn the Income Tax Depa-tment along with a certificate ofpayrnentdtaxes for the last th ree yea rs to date. 6. Particulars of specified construction equipments / machinery available with the firms / contractors with verified ownership documents. Especially mention equipment with the application intends to deploy for the works. Application should also include the yeraly increased in T&P in the last three years. 7. List of equipment with its No. !make! model /condition and location along with proof of itsownership. 8. i. Permanent business management, financial management and technical staff. ii. List of Graduate Engineers, Sub-Engineers, Surveyors, and Draftsman on the roll of firm (with proof) along with their experience and standing with the company / firm and appointment letter, proof of payment of sala ry of technical / clerical staff. 9. i. The application must establish its fina ndal sou nd ness and a certificate from Chartered Accounts firm Copyof upto ote balacesheet should also be partof the application. ii. Bank certificate and bank statement dea rly indicating financial strength of the firms/ contractorsfor last 3-years. iii. Bank documents to establish financial bid capacity 10. Details of any Arbitration/ Litigation or similar proceedings against any GovemmentorSemiGovemmerlDeparlment, showing extent and result. 11 Undertaking on judicial paper that the firm was never black listed by any of the GovemmentorSemi Govemmentorigination. RULES AND REGULATION. 1. Any concealment about the information /detail mentioned above will result indisqualification of the firm. 2. in case of incomplete information, prequalification application will be rejected and wi I not be considered for pre-qualification. 3. Applicant, firms/ contractors are required to give undeptaking that they have carefully studied the prequalification notice and it will abide by the rules and regulations. NOTE:- A C.D.R Rs.5,000/- in I he shape of Deposit at Call (non-refundable) in the name of Executive Engineer, Public Health Engg. Division, Okara be attached with the application as processing fee.
