GOVERNMENT TEACHING HOSPITAL SHAHDARA LAHORE 
TENDER NOTICE 
Sealed tenders are required from the Health Department licensed manu-facturers, importers and sole distributors for the purchase of disposable surgical items, allopathic medicines, equipment, instruments & machinery under PPRA Rules-2009 for Government Teaching Hospital Shadara, Lahore. Tender forms along with complete detail/terms & conditions can be obtained from Government Teaching Hospital, 8-Km from GT road, Shandara Lahore, against payment of Rs.1000 (non-refundable) tender fee in the following dates during working hours. All the tenders will be received by 11:00 am of the dates given below and opened the same day at 11:30 am in the Conference Room. The Committee reserves the right to cancel any or all tenders without assigning any reason thereof and this step will not be challenged in ally court of law. The firms pre-qualified by Government of the Punjab or Pakistan can take part in the tender. All the information relating to pre-qualification and application form, along with evaluation criteria, can be had from the office of undersigned.

TERMS & CONDITIONS  1. A bid security at the rate of two per cent in favour of Medical Superintendent, Government Teaching Hospital Shandara, Lahore as CDR must be attached with the Financial Offer in compliance with Rule No. 25 of PPRA Rule 2009. (Previous reference will not be accepted). 2. The tender purchase receipt will have to be affixed with the tender as a proof. 3. In case of authorized representative a separate tender form will have to be purchased for each firm and the proof of authorization append-ed. 4. Tender must carry the attested copies of Income Tax & Sale Tax reg-istration and their numbers on it. Moreover, all the rates offered in ten-der must be inclusive of all the prevalent taxes. 5. The complete detail of firm as per the conditions given in the tender, besides an authority letter, will have to be included in case of author-ized representative. 6. The tenderer's representative must bring the authority letter and iden-tity card along at the time of tender opening. 7. The successful tenderer will be bound to supply equipment by the due date. Otherwise, proceedings will be initiated against him. 8. The successful tenderer, whose rates are approved, will be bound to supply all the items to hospital at his own expense. 9. Tender will only be acceptable in printed form. Hand-written tender will not be acceptable. 10. As per Rule No.39 of PPRA Rule 2009 1he successful tenderers will have to submit a performance guarantee at the rate of five per cent of the total supply order cost in favour of Medical Superintendent as CDR. 11. Whether the supply will be made through LC or FOR be also written on the tender. 12. Adherence to time is essential. SPECIAL INSTRUCTIONS  1. All the tenders will be received under PPRA 2009 Rules. 2. As per Rule 36 of PPRA the Technical & Financial Offers must be sent separately with Technical & Financial Bids written on the top of enve-lope. Both these sealed envelopes are enclosed in a large sealed envelope.
