TENDER NOTICE
Sealed tenders are invited from manufacturers and importers for the supply of the Cardiac Catheterization Laboratory (equipnient). Tenders should reach to The office of the undersigned through registered mail I courier service on or before 22-01-2014 (12:00 tioon) and will be opened on 23-01-2014 at 1000 a.m. in the presence of bidders or their representatives. Bid security/earnest money @ 2% for firms already pre-qualified with Director General Health Services, Khyber Pakhtunlchwa, Peshawar and 5% for post qualified firms,. of the total bid. value in the shape of call deposit is required to be submitted with bid in favour of Chief Executive; Saida Teaching Hospital, Swat. The quoted rates should be on F.O.R basis. TERMS AND CONDITIONS: 1. The rates approved will remain affective till 30-06-2014. 2. Bidding will be conducted through single stage-two envelopes (one envelope of Technical Specifications and second envelope of cost) bidding procedure as per rule 40 of the Khyber Pakhttudchwa Procurement of Goods, Works & Services Rule-2003. 3. A detailed list of specifications of required items can be obtained from the purchase cell of Saida Teaching Hospital. Swat during working hours. 4. The tenders must be submitted along with the call deposit as mentioned above; otherwise the tenders Will not be acceptable. Cash / cheque and reference of old call deposit will not be acceptable-5. The original brochure must be attached with the bids. 6. The bidder will furnish address of workshop with a list of workers and landline telephone number in the tender documents. 7. The bidder will be bound to ensure the supply within 60 days for local items and 120 days in case of imported equipment or as early as possible on receipt of order, otherwise the purchase will be made on risk and cost of successful bidden. 8. f the supply' is not done within the period mentioned in the supply order, penalty @ 2% will charged up to 15 days and beyond 15 days the penalty will be charged @ 5%. 9. If the bidder is involved in any court affairs/dispute with Saidu Teaching Hospital, Swat/Health Department, his tender will not be considered. 10. The income tax, stamp duty and professional tax or any other Govt tax will be charged as per rules. 11. No conditional tender will be acceptable. 12. The successful bidder will be bound to execute agreement on a stamp paper. 13. If the Successful bidder supplies substandard item and not matching with specifications, issued from this office, it will be cancelled automatically. 14. Tender giving company must attach the relevant documents along with their tenders that should show.theit company is manufacturer of that item. 15. Prequalification of Director General Health Services, Khyber Pakhtunkhwa, Peshawar, Income Tax, Professional Mx and all other required documents must be attached with the tender. 16. Only typed tenders on original letter pad, sealed and Sighed (every page) should be submitted, hand written tenders would not be acceptable. The tenders must be according to hospital specifications.
