TENDER NOTICE (2nd)
Sealed tenders are invited from manufacturers and importers for the-supply of 128 Slice CT Scan Machine". Tenders should reach the office of the under-signed through registered mail / courier service on or before 09-01-2014 at 10.00 am which will be opened on the same day at 11:00 a.m in the presence of bidders or their representatives. Bid security / earnest money @ 2% (for firms already pre-qualified with Director General Health Services Khyber Pakhtunkhwa Peshawar) and 5% (for post qualification of firms) of the total bid value in the shape of call deposit is required to be submitted with bid in favour of Chief Executive, Lady Reading Hospital, Peshawar. The quoted rates should be on FOR basis.
TERMS AND CONDITIONS: 1. The rates approved will remain effective till 30/06/2014: Only typed tenders on original letter pad, sealed and signed (every page) should be submitted. Hand written tenders would not be acceptable. The tenders must be according to hospital specifications and alternate rates will not be acceptable. 3. The tenders naust be submitted along with the call .deposit as mentioned above, otherwise the tender will not be acceptable. Cash / cheque and reference of old call deposit will not be acceptable. The CDR must be from the Bank of Khyber. However, this condition will not be mandatory where the Bank of Khyber having no branch in reasonable area. 4. The original brochure must be attached with the bid. 5. The bidder will furnish address of workshop with a list of workers and land line telephone number in the tender documents. The bidder will be bound to ensure the supply within 60 days for local items and 120 days in case of imported equipment or as early as possible on receipt of order, otherwise the purchase will be made on risk and cost of successful bidder. If the bidder is involved in any court affairs, dispute with LRII / Health Department, his tender will not be considered. The income tax, stamp duty and professional tax or any other government tax will be charged as per rules. However, this hospital is exempted from the general sales tax. 9. No conditional tender will be acceptable. 10. The successful bidder will be bound to execute agreement on a stamp paper. 11. A detailed list of specifications of required item can be obtained from the purchase cell of LRH during working hours. 12. If the successful bidder supplies substandard item and not matching with specifications issued from this office, it will be cancelled automatically. 13. Tender giving company must attach the relevant documents along with their tender that should show their company' is manufacturer of that item. 14. Prequalification with D.G Health Khyber Pakhtunkhwa, Income Tax. ProfesSional tax and all other required documents must be attached 'with the tender. 15. If the supply is not done 'within the period mentioned in the supply order, penally @ 2% will be charged up to 15 days and beyond 15 days the penalty will be charged @ 5%. 16. Bidding will be conducted through single stage - two envelopes bidding procedure as per Ride 40 of. the Khyber Pakhtunkhwa Procurement of Goods. Works and Services Rules - 2003. 17. The purchase committee has the right to accept or reject the tender pay or totally and at any stage without giving any reason.
