District Headquarters Teaching Hospital Sargodha invite sealed tenders on the basis of annual contract rate for the purchase Of drugs/medicine/dialysis, sur-gical disposable items, lab kits and chemical, tibbi medicine, home medicine etc. Interested bidders can have tenders along with terms and Conditions from the office of the undersigned on payment of fixed tender fee of Rs 1000/-(non refundable) during working hours by 07-01-2014. Tenders 'will be received till 11.00 am Of 07.0 L.2014 and opened the same day, 07.01.2014 at 11.30 am in the presence of tenderers or their authorized representatives before the put-chase committee. COMmittte reserves the tight to reject any or all the tenders without assigning any reason thereof. Decision of the committee will be final and non challengeable. Terms and conditions:  1. Manufacturers and importers/sole agents of foreign principals of drugs/medicine/dialysis. surgical disposable items will be eligible to:sub-mit tender. 2. For lab kits and chemicals, tibbi medicine, home° medicine firms regis-tered With income tax/sales tax department will be eligible to tender. Offered rates be inclusive of all prevalent taxes. No excuse will be accept-able later on. 4. Incomplete and tenders received after due-date/time will not be acceptable. 5. Rakes be quoted on FOR basis which will be applicable for one year and tendereit will be bound to supply on the given rates. Purchase Committee reserves the right to reject any or all the tenders without assigning any reason thereof. Decision. of the committee will be final. It will not be appeal able at any higher forum or before any court of litoothel tax/ Sales tax will be deducted at the time of payment of bill as per law. Following documents must be attached with tender:  I. Tenderer should attach alleged copy of CNIC issued by the NADRA. 2. Firm should attach registration certificate of 'Income tax/ Sales tax depart-ment. 3 Firm submit an affidavit: on Rs.100/i samp paper that the firm has not been black listed. 4. Financial bid must accompany 2% security as earnest money. 5. Technical offer & Financial offer should be: in separate envelopes as per PPRA Rules (36). Technical offer & Financial offer also be written on both envelopes.
