Tender Notice For Hospital Pharmacy (2nd Time)
Ayub Teaching Hospital (ATH) is a tertiary care 1000-bed-ded hospital and providing specialized health delivery serv-ices to the millions of people of Hazara Division and adja-cent areas. The Pharmacy of ATH is being run at central place in the building on monthly rent basis. Sealed postal tenders are invited for running of F'armacy shop during the period from January 2014 to June 2015 from well reputed, financially sound parties having enough experience in the relevant field and registered with the income tax and sales tax department on the following terms and conditions: 1 The tender documents can be obtained from the Accounts Branch of ATI-I on depositing Rs 2,000/- in relevant receipt account (non-refundable) up to 01-01-2014 during working hours. 2. The firm concerned should submit the following documents with the tender (a) B. Pharmacy License (b) Last five years bank statement, showing financial position and status. (c) Copy of CNIC. (d) National Tax Number. (e) Proof of income tax and sales tax registration. (f) NOC / clearance of income tax stating NTN and GST numbers. 3. The successful bidder Shall give 5% discount to patients on retail price of medicines and other material. 4. The offer shall be accepted on the highest monthly rent basis. 5. The conditional tender will not be accepted. 6. The tender having over writing plus cutting shall be rejected. 7. The tender shall be accompanied by a Call Deposit of Rs. 2 million from the nearest scheduled bank of Pakistan (Abbottabad Branch) as earnest money. Without call deposit tender will not be entertained. Sealed tenders duly completed in all respect must reach to the undersigned on Or before 02-01-2014 by 10:00 a.rn and shall be opened at 11:00 a.m in the presence of bidders or their authorized agents who care to be present. 10.
