PRE-QUALIFICATION NOTICE
Applications are invited for- the pre-qualification for the schemes as below. for technically and. Financially capable contractors/firms whahave been enlisted/renewed with HUD & PRE Departments for the year 2014-15. having excellent reputation in the construction of large scale Water Supply and Sanitation Projects (PEC Category CE 9 and CE 10) to undertake the projects:-

The willing contractors/firms falling in the category of estimated cost of the project are required to submit the applications along with the following information/documents for pre-qualification for _above mentioned schemes up to 06-11-2014 at 2.00 PM in the office of the Executive Engineer Public Health Engineering Division. Okara.

QUALIFICATION CRITERIA AS PER PPRA GUIDELINES. 
1 Name of firms/contractors, partnership deed/articles of association in case of company/limited firm, complete- address and telephone No. Of persons with power of attorney in fiwour of person authorized by the firms/contractors. 2 Attested-copies of enlistment/renewal for the year-2014-15 with HUD & PHE Department in category 4C-4 and/or higher (Rs. 150.00 Million and above and registration of Pakistan- Engineering Council in specification Category CE 09- Cr, 10 (Water Supply & Sewerage/Drainage and General Civil Engineering: Works/Project) for the calendar year 2014. 3 i Year 'of establishment:of firm/company with proof.

ii No. of similar nature works, cost of each work (equal Or .greater amount) scope of work with verification from concerned Executive Engineer. NaMe of similar projects already completed during last. three years with the completion certificates form the concerned office and number of projects presently in hand along with their magnitude and period and report about quality & space or work from the client Department.
4 Proof of financial standing:41y supported with documentary proof/bank cert-tificate (Balance sheet duly certified by the Cemented bank officer be pro-dived).
5 Proof of registration with the income Tax .Department along with :a certificate of :payment of tax for the last: three financial years..
6. Particulars. of Specified construction equipments/machinery:available with the firms/contractors with verified ownership  documents. specially mentioned equipment with the application, intends to be deployed for the work/works. Application should also include the yearly increased in T&P in the last three years.
7 List of equipment with is No./make/model/condition and location along with proof of its ownership.
8- i Permanent buSitieSS Management, financial than age then and technical staff. ii List of Graduate Engineers,

Sub Engineers, Surveyors, and Draftsman: on the roll of firm (with: proof) along with their experience 'and:Standing with the company/firm and appointment letter, proof of payment 'of salary -of techni-cal/clerical staff. 9
 i The application must established its financial soundness and a certificate form Chartered Accounts firm. Copy of up to date_ balance sheet should Asci-i part of the 'application.
ii Bank certificate Hand Bank statement clearly indicating financial strength of the rims/contractors for last 3 years.
iii Bank documents to establish financial bid capacity. 10 Details of any Arbitration/Litigation or similar proceedings against any Government or Semi Government Department. showing extent and result.
11: Undertaking on judicial paper that the firm was never blaCk listed by any. of the Government or Semi Government origination.
11 The interested firms should .obtain Pre-qualification'.
12.Performance documents-from the Head Clerk of the undersigned and submit duly filled & signed/stamped along with- the application and requisite documents.
13 The Applicant must enclose C.D.R/G.R of Rs. 2000/- (non refundable) under rule 1741)- of PPRA rule for project separately in the name :a Executive Engineer, Public Health Engg: Division Okara for documents processing fee. The contractor change tender documents before 06-11-2014. 14 All PPRA Rules shall be strictly followed.
RULES. AND REGULATION:-. 1 Any Fake document S & concealment about the information/detail Mentioned above will result disqualification of the firm. 2 In case of incomplete information, pre qualification will reject the application and it will not be considered for pre-qualification. 3 If the Performance's not attached duly filled with the applicatiOn/ pre qualification documents it will not be entertained for processing of prequalification.
4 The Decision of Pre-qualification cam:mince: will be final.
5 Applicant firms/contractors are required to give under taking that they have carefully studied the pre qualified notice will abide by the rules and insulation.
6 Each page of document provided dttly 'numbered, singed and attested: by the applicant, documents attached according to serial no. as mentioned aboVe an index with page.No. be provided.
7 Information. to firm regarding pre-qualification will be given. after approval form the committee constituted by the Chief Engineer (South) Punjab. Public Health. Engineering Department. Lahore. the -competent authority.
