Tenders are invited for the purchase of MRI Machine (Along with Civil Work) for Hayatabad Medical Complex Hospital Peshawar on the basis of FOR and C&F during 2013-14 from the manufacturers, importers (registered for specific it registered with Director General Health Services Khyber Pakhtunkhwa. The tender should be sent through registered post / courier to the office of Medical Superintendent HMC, Peshawar. The tender will be opened on 28.01.2014 at 11:00 am in the presence of bidders or their representative in the office of the under signed. The items is :-  S.No Name of Items  1 MRI Machine (Along with Civil work).  Terms & Conditions. I). The rates approved will remain effective till 30.06.2014. (2). Only those firm can apply for the tender who are Registered with Director General Health Service KPK, Peshawar for the year 2013-14 or having previous year DGHS registration. (3). The firm can only apply for the category in which the firm is registered. (4). Only typed tender on original prescribing letter pad sealed & signed should be submitted, hand written tender would not be accepted. (5). Original broachers should be attached with the tender documents. Moreover country of origin should clearly be mentioned in quotation. (6). The bidder will be bound to ensure the supply of the said order within stipulated period otherwise penalty will be imposed as per rules. The competent authority having the right to extend the suplly period if needed. (7). Proprietary certificate must be attached if necessary. (8). Firm applying for the tender having Main / Branch office in Peshawar will be preferred. (9). If the bidder is involved in any court affairs, dispute vvith HMC his tender will not be considered. (10). The taxes will be deducted as per rules. (11). No conditional] / optional tender will be acceptable. (12). The successful bidder will be bound to submit the prescribe agreement on an stamp paper of Rs.100/-. (13). 5% call deposit must be attached with the tender in case non registered firm othenvise 2% call deposit should be attached with the tender 10% Bank Security should be obtained after issuing supply / woth order / receipt of equipmenm. (14). No tender document will be issued on opening date. (15). The Purchase committee has the right to accept or reject the tender partially or totally and at any stage, without giving any reason. (16). If the successful bidder supplies substandard & not matching with specification in tender documents issued from this office will be cancelled automatically.(17). D.G Registration, Sales Tax Certificate / Professional Certificate and all other required documents must be attached with the tender. (18). Detailed Specification of MRI Machine / civil work will be available in the office of the undersigned. (19). Technical and Financial quotation will be submitted separately in separate envelopes. (20). Tender documents should be supported with the table of contents (Book Shape).
