Department and have got their names registered/renewed with Pakistan Engineering Council and Communication and Works
Department for the year 20132014 for pre~qualification for tendering for the following works. Contractors!-' inns who have
already applied need not apply again.

The interested contractorsl firms are required to submit following information I docu ments duly page marked and indexed
(in duplicate) to the office of the undersigned so as to reach by 18.02.2014 during office hours. Separate Application for each
work submitted.
1. Name and full postal address and partnership deed of the contractors/firms with power of attorney in favour of person
authorized.
2. Year of Establishment of the firm supported by certificate from the Registrar of Firms.
3. Name and particulars of specialized firms to be associated.
4. List of cases pending in Arbitration /Litigation if any.
5. Certificate of Registration from Pakistan Engineering Council Islamabad in the relevant category for the current
calendar year (duly attested by the First Class Officer).
6. Copy of Enlistment/Renewalfortheyear2013-2014with C&W Department in the relevant category.
7. List of complete permanent business management staff. Finance Management staff and Engineering!
technical staff (such as Graduate Engineers, Sub Engineers- Surveyor/Draftsman elc)with their complete Bio Data ilc
experience and proof of stay with the firm.
8. List of equipment and machinery especially suitable to the type and volume of this project with its
No./Make/Modev condition and location along with the proof of ownership of machinery.
9. Detail of similar projects costing more than or equal to the cost of the projects completed by the contractor/firrn giving
location. approximate cost, time taken for completion duly supported with certifite from Client Department.
10. List of projects handled during Iast three years giving their location, approximate cost time allowed /taken for completion
duly supported with certificate from Client Department.
11. Performance certificate from the Executive Engineer client under whom the works have been executed ‘during last
three years and wrtificate to the effect that funds for the works were not lapsed /surrendered due to slow progress of
contractor during last three years. In case of non-production of this certificate, the applications are liable to be rejected.
12. Detail of work in hand indicating name of Client Department. Consultant. scope of work com pletedl
payments received upto date by the Contractors! firms.
13. Authentic proof of financial position such as Bank Statement of previous one year.
14. Total assets. work capital and liabilities duly certified.
15. lncomeTax Registration Certificate.
16. Any further particulars of the firm which the Fi'm wish tofumish may also be attached.
17. Each booklet should be duly page marked and index foreach content be provided at the first page.
18. The applications/bookies without index and page marking are liable to be rejected.
Pre-qualification application shall be evaluated onthe basis of Planning and Development Department criteria for
pre-qualification. The other related information required in this regard should also be provided. Any further information/detail
in this connection may be obtained from the office of the undersigned or Executive Engineer Provincial Building Division
Rawalpindi on any working day or from website of Government of the Punjab. Only per-qualilled Finns willbe invited through
letters on their postal address to submit tenders.
