PRE QUALIFICATION NOTICE
PUBLIC HEALTH ENGINEERING DEPARTMENT RAHIM YAR KHAN
Public Health Engineering Division Rahim Yar Khan Invite's applications for pre qualification of contractors/firms for the work mentioned below from reputed contractors/firms having sufficient experience in relevant work and duly enlisted/renewed for the year 2014-15
HUD& PHE Department.

Name of work Amount: in Millions 1 Sewerage scheme Rahim Yar Khan City with treatment plant. Construction of intermediate pumping station near Niazi Colony, installation of force main and laying of Sewer line.
Amount: 405.000 Millions
The interested firms are required to submit the following information/documents (in duplicate) to this office on or before 20-06-2015 during office working hours.

1. Year of establishment of firm.

2. Name of firm along with postal address & telephone numbers.

3. Name with full address & partnership deed of the contractor/firm with power of Attorney in favor of person authorized. 4. List of technical staff with their Bio Data and proof of stay with the firm.

5. List of equipment with make/model, condition and location along with the proof of its ownership.

6. Soundness of financial position such as Bank Certificate of previous three years.

7. Detail of similar projects completed during the last three year, location, cost, time taken for completion duly supported with certificates from client Department.

8. Detail of work in hand indicating the client Department, consultants, scope of work cost, date of start & expected date of completion by the contractor/firm.

9. List of cases pending in Arbitration/ Litigation, if any be attached.

10. Certificate of Registration from Pakistan Engineering Council, Islamabad , for 2015 in the relevant category (above 400 Million) including Water supply & Sewerage code (CE-10 & CE-09) duly attested by 1" Class Officer.

11. Copy of Enlistment/Renewal for the year 2014-15
HUD&PHE Department.

12. Performance certificate from the Executive Engineer under whom the works have been executed during last three years.

13. Income Tax Registration Certificate (NTN).

14. Affidavit regarding non black lists the firm by any department. Prequalification documents duly signed by undersigned can be obtained from the office of undersigned during office hours, after publication of this notice and after payment of Rs. 2000/- as publication and printing charges.
Only those Prequalification applications will be considered which are submitted according to Prequalification documents issued by undersigned and accompanied by receipt of fee received for issuance of pre qualification documents.
