WHEREAS the under signed (authority) is of the opinion that it is not necessary to hold inquiry into the matter that while working as Medical Social Worker Sahnval Medical College. Sahivral you committed irregularities as underneath You went to Sargodha without intimation and prior permission of the competent authority on 03-12-2011 You are absent from duty since 03-12-2011 till date You produced a Medical Certificate issued by Dr. Sharafat Ali Eye Specialist / Surgeon Police Line Road. Sahnval (Private Doctor) in support of your illness with effect from 06-12-2011 which indicates that the doctor concerned advised you three days bed rest with effect from 0622-2011 whereas you have not joined duty after expiry of your bed rest iv You were directed to appear before the Medical Board on 18-01-2012 at 10 00 AM along with all relevant documents which you intend to produce in support of your illness vide this office registered mail letter No 1071ESTT/SMOSVVL dated 14-01-2012 but you did not turn up for required purpose v. Two (02) Show Cause Notices bearing Nos. 219/ES11'/SMCISWI. dated 21-02-2012 and 493/ESTTISMC/SWL dated. 18-04-2012 and subsequently a final Show Cause Notice vide this office No 7861ESIT/SMC/SWL dated. 19-06-2013 was issued to you on your above mentioned address, but you did not appear for duty.
AND WHEREAS it is not necessary to conduct formal inquiry of the absence period, the undersigned as authority in your case has determined that the charge of unauthorized absence from duty against you stand proved NOW THEREFORE you are hereby called upon to show cause in writing within seven (07) days of the publication of this Notice tailing which it shall be presumed that you have no defense to offer or you have declined to offer the same and major penally of "Dismissal from Service" under the Punjab Employees Efficiency. Discipline and Accountability Act, 2006 shall be imposed upon you. In case you desire to consult any record on which the aforesaid charge s based or is relevant to, you may do so with the prior arrangements with the undersigned within prescribed time limit
