Sealed tenders ate invited from the Mantfadturers and Irnportere for the Sapply of Medical Gases & Related Iteins. Tender should reach to the office of the undersigned thro4tRegisteted mail/Courier Service on or before 07.-01-2014 at 10.00 AM which will be opened on the same day at 11 AM in presence of bidders or their representatives.
TERMS AND CONDITIONS: 1. The rates approved will remain effective till 31/12/2016. 2. Only typed tender on original letter pad, Sealed & signed (every page) should be submitted, hand written tenders would rtot be acceptable. The tenders must be according to hospital specifications; alternate rates Will Et. be acceptable. 3. The tenders must be submitted along with the can deposit of Rs10,00000/- othetwise the tender will not be acceptable. Cash / cheque and reference of old call deposit will not be acceptable. The CDR must be from the Bank of Khyber. However, this condition will not be mandatory where the Bank of Khyber having no branch at reasonable area. 4: The original brochure must be attached with the bids. 5. The bidder will furnish address of workshop with a list of workers and land line telephone number must be mentioned in the tender documents. 6. sample of the quoted itetts should be produced by the bidder on the date of purchase committee meeting. where neeessary. 7: The bidder will be bound to ensure the supply of Medical Gases as and when required immediately and other items-within 60 days for local items and 120 days in case of imported items or as early as possible on the receipt order, otherwise the supply will be made on risk and cost of successful bidder. 8. If the- bidder is involved in any court affairs, dispute with LRI-I/Health Department, his tender will not be considered. 9. The Income Tax, stamp duty and Professional Tax or any other Cosi* tax will be charged as per rules. 10. No conditional tender will be acceptable. 11. The suceeSSfdl bidder will be bound to execute agreement on a stamp paper. 12. A detailed list of specification tithe required item,s-can be obtained from the Purchase Cell of LRH during working hours. 13. If the supply is not made within the period tnentioned in the supply order, penalty 2% will be charged uptc.)15 days and beyond 1.5 days the penalty will be charged @ 01 5%. 14. The services of Medical Gases Will be provided by the company as and when required free of charges. 15. Bidding will be conducted through Single stage -two envelop bidding procedure as per Rule 40 of the Khyber Paldittinithwa Procurerrent of Goods, works and Services Rules-2003. 16. lithe successful bidder supplies substandard and not matching with specification issued from this office will be cancelled automatically. IT Registration with D.G Health Khyber Palchtunkhwa, Income Tax, Professional tax and all other required documents must be_ attached with the tender. 18. The purchase committee has the tight to accept or reject the tender partially or totally and at any stage, without: giving ,any reasons.
